Our scheduling office is available Monday through Friday 8:30 AM – 4:30 PM. We are closed most holidays. Our staff can schedule your appointment for either PHGI Associates or the Washington Square Endoscopy Center.

We appreciate the opportunity to serve you and value your time. To expedite the scheduling process, please have the following information available when you call:

• The reason for the appointment
• The name and contact information of your referring physician
• Current insurance information

Please notify our schedulers if you have any special physical or cultural needs.

If you have been mailed an information packet, please be sure to complete any forms, as directed, and bring them with you at the time of your visit.

You will be asked to present photo identification and current insurance information at each visit.

It is our policy to call your name when providing services within the office. Please notify our staff if this does not meet your needs.
