Insurance and Billing Information:

We participate in most insurance plans and will file your insurance claim for you. Although we participate with most insurance plans, be sure to check with your insurance carrier for specific benefits.

To ensure accurate billing, you will be asked for current insurance information each time you check in for an appointment at either the office or the endoscopy center. You will be asked to review all your personal information upon check in. Please review this information carefully and alert our staff of any changes.

The Washington Square Endoscopy Center is a licensed and accredited ambulatory surgical facility. Just like a hospital, there is a fee for the use of the facility, the personnel and equipment. Physicians who have been granted privileges by the governing body may perform procedures in the center. When you have a procedure performed in the center you will receive more than one bill. One statement will be from the physician who performed the procedure – a professional charge. A second statement will be from the endoscopy center – a facility charge. Also, if a biopsy is performed or anesthesia is administered, you may receive additional statements for these services.

If you are scheduled to have a screening colonoscopy, you may wish to verify your insurance coverage for both screening and diagnostic procedures before your appointment. This may change your coverage and / or out-of-pocket expense.

Many plans require a referral for services, an authorization form or number should be brought from the primary care physician’s office. Also, many require precertification for outpatient procedures. If you do not know whether your insurance requires precertification, closely examine your cards and check with your insurance provider.

All copays are due at the time of your appointment.

We accept all major credit cards, personal checks and cash.

You are responsible for paying any deductible, co-insurance, or other balance that is not paid by your insurance company.  Accounts are due upon receipt of your first statement.
